What we do

Our Dementia, OPMH and Frailty and Carers Teams work closely with our partners in local Primary Care Networks across Castle Point and Rochford. We work together to provide a holistic community service between the GP and Community Health Teams

Ageing well doesn’t just apply to older people. Therefore, looking after our health during our younger years is important to have the best possible chance of staying well as we get older and remaining independent for as long as possible. We aim to support, enable, and empower people to age as well as possible, regardless of their circumstances and encourage them to plan and prepare for the later stages of life from an earlier age.

The EPUT Dementia, Older People’s Mental Health and Frailty Service in Southeast Essex is made up of teams that sit within one Service. We support people of any age with suspected or diagnosed dementia, those who require specialist mental health services in later life and those who are frail.

We work closely with our partners in Primary Care, Community Health and Adult Social Care, Southend Hospital and Voluntary & Community Groups to provide a wraparound service for those who may be frail, need cognitive, memory or mental health support.
